Immunizations and Health Insurance


Certificate of Immunization

  • The US Department of State requires all F-1 students to complete and submit 3 immunization forms: Certificate of Immunizations, Medical History, and Tuberculosis Screening Questionnaire.
  • The Certificate of Immunizations is mailed with your immigration materials or can be printed from Health Services website.
  • Once you have registered for classes (during SOAR for undergraduates), you will have access to your My.GeorgiaSouthern account and will be able to upload the Certificate of Immunizations using the Eagle OSH (Health Center Online).  At that time you will be prompted to complete the other 2 forms.

Health Insurance

The U.S. Department of State requires all F-1 students to maintain continuous health insurance coverage. The University System of Georgia (USG) has extended this requirement to F-1 international students and offers a Student Health Insurance Plan (SHIP) through United Healthcare. You will be automatically billed after you register for classes every semester. The health insurance fee, paid in August, covers August through December. The fee paid in January covers January through July. During orientation, we will discuss how the health care system works in the United States and how to use this health insurance plan in the event that you require medical attention. Please bring any questions you may have to orientation!

All international students (with F-1 visa) are required to purchase the University System of Georgia Student Health Insurance Program (USG SHIP) unless they have insurance which meets the university system requirements.
