Deadline for Appeal: Students are encouraged to initiate the appeal process as soon as they are aware of their dismissal.  Appeal Form must be received in the Academic Success Center office by noon the day before classes begin.

Write a brief paragraph explaining any unusual circumstances which resulted in your dismissal from the University and why you should be granted an additional semester

Completion of this form does not guarantee that you will be granted an additional semester.  Additional semesters are the exception rather than the rule and are granted on a case-by-case basis. If granted the additional semester attempt, the student may take only the required learning support course unless the committee approves additional course work.
