Instructions - Faculty Information Data (FID) Form

Units submit a completed Faculty Information Data Form (FID) for all new faculty appointments, including regular faculty, temporary, part-time, adjunct, and non-tenure type appointments. The FID is also used for appointing new graduate teaching assistants.

  • The form is a Microsoft Word document which may be downloaded and submitted with the appointment packet or sent electronically to rtbowen@georgiasouthern.edu. Access the form at http://academics.georgiasouthern.edu/provost/domain/fid.doc on the Provost's Office website (GSU network access only).
  • All dates should be entered in a MM/DD/YEAR format.

  • When the major subject area of the faculty member’s highest graduate degree differs from the teaching field in which the faculty member will be teaching, the dean should send a brief justification explaining why this faculty member is qualified to teach in this subject area to the Provost’s Office along with the appointment package or submit the justification electronically to candaceg@georgiasouthern.edu.
