Instructions - Faculty Information Data (FID) Form
Units submit a completed Faculty Information Data Form (FID) for all new faculty
appointments, including regular faculty, temporary, part-time, adjunct, and
non-tenure type appointments. The FID is also used for appointing new graduate
teaching assistants.
The form is a Microsoft Word document which may be
downloaded and submitted with the appointment packet or sent electronically
to rtbowen@georgiasouthern.edu.

on the Provost's Office website (GSU network access only).
All dates should be entered in a MM/DD/YEAR format.
When the major subject area of the faculty members highest graduate
degree differs from the teaching field in which the faculty member will be teaching,
the dean should send a brief justification explaining why this faculty member
is qualified to teach in this subject area to the Provosts Office along
with the appointment package or submit the justification electronically to candaceg@georgiasouthern.edu.

Tenure Status:
- Tenured only assistant, associate, and full professors who are
on full-time, regular appointments are eligible for this designation
- Non Tenure on Track all assistant, associate, full professors,
and instructors, visiting, and temporary faculty on full-time appointments
are eligible
- Non Tenure Not on Track includes a handful of positions not eligible
for tenure or probationary credit (i.e., full-time administrators with faculty
rank, positions funded from soft money, clinical positions)
- Not Applicable all adjunct faculty (honorary) and part-time faculty
appointments should be listed under this category as well as teaching graduate
assistants

- Per course part-time faculty (salary should represent per course
rate)
- Per semester part-time faculty paid at other than a per course
rate and full-time, temporary faculty appointed for only one semester (salary
should represent semester rate)
- Not Applicable (e.g., retirees, teaching graduate assistants, adjunct
faculty)
Degrees: please list from highest to lowest. Indicate the country if
the degree-granting institution is located outside of the U.S.
